I’ve heard tell of a range accident book with the eerie simple entry “Dog retrieved grenade”…
Quite the range of incidents…
at least a few had good recoveries…

I don’t know if there is an online video for it, since I watched the recording during a class at the Fleet Combat Training Center Pacific back before YouTube was a thing, but it was several minutes of the deployment of an anti-helicopter torpedo. A destroyer was conducting a test shot, with a helicopter about fifty yards off its beam as the torpedo was fired; the torpedo suffered some sort of major gyro failure, and came up out of the water underneath the helicopter, making it about halfway to it (the pilot, no idiot, climbed a couple of hundred feet in response) before falling back into the water; it made three more jumps at the helicopter, all of decreasing height, before running around on the surface for several minutes until it ran out of fuel.
